Making the Case for Same-Day Dentistry: Milling, 3D Printing and the Modern Chairside Workflow

For years, same-day dentistry has been positioned primarily as a technology story.

Buy a scanner. Add a mill. Invest in a 3D printer. Manufacture a restoration in the office.

But that's not really the reason same-day dentistry matters.

The more compelling story is what happens when you eliminate an entire appointment from a patient's treatment—and what that can mean for the patient, the dental team, and the practice.

Today's dentists also have more ways to build that workflow than ever before. Chairside milling remains a proven option for producing permanent restorations, while advances in restorative 3D printing are creating an entirely new approach to same-day manufacturing.

So the question is becoming less about whether same-day dentistry makes sense and more about which same-day workflow makes sense for your practice.

Patients Have Been Asking for Convenience for Years

The demand for single-visit dentistry isn't new.

In a 2015 Exevia GmbH survey of 717 dental patients:

  • 85% said they would prefer single-visit dentistry
  • 50% said they would be willing to pay more for single-visit treatment
  • 67% said they would travel farther to receive it

The takeaway: patients place significant value on convenience.

Think about the traditional crown experience from the patient's perspective.

They schedule an appointment, receive anesthesia, have the tooth prepared, leave with a temporary restoration, then rearrange their schedule again to return for delivery.

With an appropriate same-day workflow, many of those steps can be consolidated into a single visit.

One appointment. One anesthetic event. No temporary. No waiting for the laboratory. No second trip back to the practice simply to seat the final restoration.

That's a patient experience that's easy to understand.

The Practice Benefits Too

Patient convenience may be the easiest benefit to communicate, but same-day dentistry can also change how the practice operates.

A traditional restorative workflow doesn't end when the patient leaves the first appointment.

The case still has to move through the laboratory process. The temporary has to survive until delivery. A second appointment has to remain on the schedule. The team has to prepare for the patient's return, seat the restoration, make any necessary adjustments, and complete delivery.

Occasionally, the temporary breaks or comes off, creating another call or unscheduled visit.

Bringing manufacturing in-house gives the practice more control over that process.

For the right cases, same-day workflows can help reduce:

  • Second restorative appointments
  • Temporary restorations
  • Outside laboratory turnaround
  • Shipping and case coordination
  • Temporary-related emergencies
  • Scheduling complexity

The value isn't simply that a machine can make a crown.

It's that the workflow can eliminate steps.

Same-Day Dentistry No Longer Means Just Milling

For a long time, the phrase "same-day dentistry" was nearly synonymous with chairside milling.

That's changing.

For practices focused heavily on permanent restorations, milling can offer an extremely versatile same-day workflow with proven materials and predictable production.

Restorative 3D Printing

3D printing has traditionally been associated with models, surgical guides, dentures, night guards, and temporary restorations.

That's rapidly evolving.

New restorative printing systems and materials are expanding additive manufacturing into permanent restorative dentistry. Instead of cutting a restoration from a block, these systems build it additively.

That creates another pathway toward the same goal: producing a final restoration in the practice while the patient is still there.

Milling vs. Printing: Which One Should You Choose?

This is where Phil's advice to practices becomes especially important:

Don't start with the machine. Start with the workflow.

There's no universal answer to whether a practice should mill or print.

Instead, consider questions like:

  • What types of restorations do you perform most frequently?
  • Which materials do you want to use?
  • How many restorations do you expect to produce?
  • What does your current scanning and design workflow look like?
  • How much space do you have?
  • What level of finishing and post-processing fits your team?
  • Which other applications would you like the equipment to support?
  • Where do you want your digital workflow to go over the next several years?

Some practices may be best served by milling.

Others may find restorative printing particularly attractive.

And increasingly, sophisticated digital practices may find a role for both.

The objective isn't to pick a side.

It's to build the manufacturing workflow that supports the dentistry you want to deliver.

The Scanner and Design Workflow Matter Just as Much

The mill or printer gets most of the attention, but manufacturing is only one part of the process.

A successful same-day workflow needs to connect:

Scan → Design → Manufacture → Finish → Deliver

If any one of those stages creates unnecessary complexity, the benefits of chairside manufacturing begin to disappear.

That's why scanner compatibility, design software, automation, material selection, post-processing, and team training all matter when evaluating a system.

The best same-day workflow isn't necessarily the one with the fastest machine.

It's the one your team can repeat predictably.

What Does Same-Day Dentistry Really Sell?

Dentists sometimes evaluate same-day dentistry entirely through the lens of equipment ROI.

That's important—but it misses part of the opportunity.

What you're really offering patients is time.

You're giving someone the ability to complete treatment without taking another morning off work.

You're eliminating the inconvenience of a temporary.

You're reducing another trip across town.

You're creating an experience that's noticeably different from the traditional restorative process.

And that can become part of how patients perceive the practice itself.

Modern technology isn't impressive simply because it's digital.

It's impressive when the patient can clearly feel the difference it makes.

Start with the Experience You Want to Create

Same-day dentistry has evolved significantly.

Practices now have intraoral scanners capable of capturing highly detailed digital impressions, increasingly automated design platforms, proven chairside milling systems, and a new generation of restorative 3D printers.

That gives dentists more choices—but it also makes choosing the right workflow more important.
